A framework for understanding your options, and choosing how far you want to go in pursuit of your health.
Not every patient comes to us wanting the same thing. Some want to resolve an immediate problem. Others are ready to go deep, to make sure their mouth is a source of health, not hidden harm. This framework is our way of making that conversation clear.
We think of dental care as a spectrum: seven distinct levels, each one building on the last. At every level, you gain something real and meaningful. The question we want you to sit with is simply this: where do you want to set the bar?
There is no wrong answer. There is no pressure. These seven levels exist so that you can see the whole picture, understand what each commitment involves, and make a decision that is genuinely yours. Our job is to meet you wherever you are and to take you as far as you want to go.
Level 1: Foundation Health
No gum disease. No decay. A maintenance plan that keeps it that way. This is the non-negotiable starting point, the foundation everything else is built on. Gum disease and decay are the leading causes of tooth loss and are linked to heart disease, diabetes, and systemic inflammation. The connection between oral health and whole-body wellness starts right here.
Level 2: A Complete Arch
Every tooth has a neighbor and a tooth to bite against. When one is lost and not replaced, the arch destabilizes: teeth drift, bone resorbs, and bite forces redistribute in harmful ways. Level 2 is the commitment to keeping every tooth accounted for, whether through implants, bridges, or partial dentures.
Level 3: Mercury-Free
Amalgam fillings are roughly 50% mercury, one of the most toxic substances on Earth. Mercury vapor is released continuously during chewing, grinding, and eating. Level 3 means having a clear, phased plan to replace them safely using SMART (Safe Mercury Amalgam Removal Technique) removal protocols and biocompatible materials.
Level 4: Functional Harmony
Your bite is part of the stomatognathic system: teeth, jaw joints, muscles, and the trigeminal nerve (the largest cranial nerve in the body). When it is out of balance, the effects show up as wear, headaches, jaw pain, and instability. Orthodontics and clear aligner therapy belong here too. Tooth position is a functional matter, not just a cosmetic one.
This level typically moves in two phases. The first is occlusal appliance therapy, stabilizing the bite and relieving symptoms. The second is definitive bite correction, addressing the underlying structural issues for a lasting result. For patients dealing with TMJ pain, headaches, or jaw dysfunction, this is where that conversation starts.
Level 5: Airway and Sleep Dentistry
The mouth and jaw are the gateway to the airway. Sleep apnea, airway obstruction, and disordered breathing affect hundreds of millions of people, most of them undiagnosed. The consequences go far beyond snoring: cardiovascular strain, cognitive decline, hormonal disruption, and chronic fatigue.
Level 5 evaluates and addresses the airway as a core component of whole-body health. This is an area where dentistry and medicine overlap in ways most patients have never been told about.
Level 6: Biological Optimization
For patients taking a whole-body approach: no metals, no dead teeth, no infected root canals, no cavitations. Using advanced 3D imaging and biocompatibility testing, we ensure the mouth is completely clear of every source of chronic infection, inflammation, or toxic burden. This is dentistry practiced as a branch of medicine.
At BioSmile Health, this level draws on our full range of natural healing modalities, including ozone therapy, platelet-rich fibrin (PRF), laser treatment, and biological oral surgery, to resolve hidden sources of inflammation and give the body a clean foundation to work from.
Level 7: Optimal Aesthetics
With health, function, airway, and biology fully established, aesthetics becomes the natural final expression. Smile esthetics refines and transforms the appearance of your teeth. Facial esthetics is something deeper: the synergistic result of all seven levels working in harmony. When everything beneath the surface is right, it shows in the face.
We would love to walk through these levels with you and build a plan that is genuinely yours.
